Margin index breast
Lumpectomy margins are the rim of normal tissue surrounding the cancer tumor in situ (DCIS) for women treated with lumpectomy and whole-breast radiation. 18 Jul 2019 The need to return to the operating room for positive margins after breast- conserving surgery delays adjuvant therapy, is upsetting to patients, Margin index, defined as the mathematical relationship between the size of the tumor and the closest margin (see Fig. 1), was shown to be predictive of the probability of residual disease following surgery with close margins for women with Stage I-II breast cancer within one large academic center 13. Margin index, a mathematical relationship between tumor size and closest margin, has been shown to be predictive of the probability of residual cancer after BCS for early stage breast cancer. We applied this tool to the same population of patients at our institution to evaluate its ability to predict residual disease after BCS.

Is radiation therapy necessary if the margins of the removed tissue are clear? How can I make a decision between mastectomy and breast conservation therapy? For patients with node-negative breast cancer who undergo breast conserving by BCS with clear margins were randomized to receive whole breast RT with 42.5 Gy in 16 fractions (n Cambridge Postmastectomy Radiation Therapy Index Invasive breast cancer after primary mastectomy ± reconstruction . recurrence ( size, margin width, pathologic classification and age). Scores for each of the predictors are totalled to yield a Van Nuys Prognostic Index score ranging from a low 15 Jun 2018 This description might report the presence of "inked" margins or sutures, which the surgeon adds so the pathologist can tell "which end is up" 23 Jun 2016 Margins that have cancer cells in them are called "positive." Because all the breast tissue is removed during mastectomy, the need for more
OBJECTIVE: We previously introduced the concept of margin index as a method for prediction of residual disease after attempted breast-conserving therapy (BCT). We sought to apply the margin index to patients with ductal carcinoma in situ (DCIS) to determine its reliability in predicting residual disease.

It is based on size & grade of DCIS, margins and age of patient. DCIS patients with scores of 4, 5 or 6 can be considered for treatment with excision only. Patients with scores (7, 8, or 9) should be considered for radiation therapy or be re-excised if cosmetically feasible.
With a margin index of 20, no residual disease was found in the reexcision specimen. Re-excision may not be necessary for involved anterior and posterior margins if underlying muscle fascia or overlying skin has been removed. If reexcision is not OBJECTIVE: We previously introduced the concept of margin index as a method for prediction of residual disease after attempted breast-conserving therapy (BCT). We sought to apply the margin index to patients with ductal carcinoma in situ (DCIS) to determine its reliability in predicting residual disease. CONCLUSIONS: Margin index is a reliable method for the prediction of residual disease after attempted BCT with close margins. This simple calculation may be helpful for identifying patients who require reexcision before radiation therapy and those who may be able to forego additional surgical interventions. The margin index is equal to the closest margin (in mm) divided by the tumor size (in mm) × 100. "Most patients were Caucasian with estrogen-receptor- or progestin-receptor-positive breast cancer," The surgeon’s goal during surgery is to take out all of the breast cancer along with a rim of normal tissue around it. This is to be sure that all of the cancer has been removed. During or after surgery, a pathologist examines this rim of tissue — called the surgical margin or margin of resection — to be sure it’s clear of any cancer It is based on size & grade of DCIS, margins and age of patient. DCIS patients with scores of 4, 5 or 6 can be considered for treatment with excision only. Patients with scores (7, 8, or 9) should be considered for radiation therapy or be re-excised if cosmetically feasible. The width of the clear margin shouldn’t affect which type of radiation therapy a woman receives. Women younger than 40 who are diagnosed with early-stage breast cancer have a higher risk of recurrence in the same breast after lumpectomy and a higher risk of recurrence in the chest wall after mastectomy.
